Appointment Scheduling
Appointments are required. Please schedule an appointment prior to arriving at the Raytown Schools Quality Care Clinic through one of the following methods:

To schedule an appointment, sign into My Premise Health online or in the app and click the 'Schedule an Appointment' icon.
To send a secure message to health center staff, sign in to My Premise Health online or in the app and click the 'Send a Message' icon. To view sent and received messages, select 'Messages' from the Menu list.

First Visit Instructions
Prior to your first visit, you will be asked to complete new patient materials.
To save time, complete the paperwork before you arrive by logging in to mypremisehealth.com or through the app. Select eCheck-in to complete all required forms and verify your health information. Click here to learn how to eCheck-in.
If you are unable to provide your information via mypremisehealth.com, please plan to arrive 10 to 15 minutes prior to your appointment time to complete all necessary paperwork.
Bring your Driver’s License and Blue KC Member Insurance Card with you to your visit. Your insurance card is also available electronically in the MyBlueKC mobile app.
Scheduling Details
Appointments are Required: Please schedule an appointment prior to arriving at Raytown Schools Quality Care Clinic. This helps ensure you and your fellow staff members have the shortest wait times and best experience possible.
Scheduling for the First Time: If you are scheduling for the first time, you must first be registered with Premise Health. Click here for instructions on how to register.
Urgent Needs: If you have an urgent need and are unable to find a same day or next day appointment, please contact Member Services to see if they can accommodate you.
Virtual Visits Only Available for Certain Appointment Types: Virtual Visits by phone or video are only available for certain appointment types such as cold, flu, follow ups, lab reviews, medical questions, allergies, and prescription refills. Click here for information Virtual Visits.
Cancellations: If you are unable to make your appointment time, please call ahead to cancel or do so online or in the mobile app, out of courtesy for other patients and the Raytown Schools Quality Care Clinic staff.
