Meet the Care Team

Raytown Schools Quality Care Clinic's care team includes a Physician, Nurse Practitioner, Registered Nurse, Medical Assistant, Physical Therapist and Behavioral Health Counselor.  All are employees of Premise Health. 

Providers

Katie Hern, MD

Physician

Dr. Katie Hern is a family physician who has been practicing in the outpatient primary care setting since 2010. She received her undergraduate and medical education from the University of Missouri-Kansas City Accelerated 6 year B.A./ M.D. medical program. She then went on to complete her Family Medicine residency through UMKC at Truman- Lakewood Hospital.  Dr. Hern has been serving the Raytown Schools Quality Care Clinic since 2015. She enjoys breaking down medical conditions and treatments in an easy-to-understand way for her patients to get "AHA" moments with them and to create lasting change in their health and wellness. 

In her spare time, she enjoys spending time with her husband and 2 young boys on their farm. 

Amber Williams, DNP, FNP-BC

Nurse Practitioner

Amber Williams is a nurse practitioner who has been working in the healthcare field since 2015 when she completed her undergraduate nursing education at Saint Luke's College of Health Sciences in 2015. She then went on to complete her Doctor of Nursing Practice with Family Nurse Practitioner emphasis at the University of Missouri-Columbia in 2021. Previously, she worked as a nurse practitioner at the University of Kansas Health System specializing in rheumatology and acute care in the inpatient setting. Amber also has experience as a nurse practitioner in the urgent care setting. 

In her spare time, she likes to go to the gym, go hiking, kayaking, bicycling. She also enjoys traveling, spending time with friends and loved ones, and exploring new restaurants and coffee shops.

Lana Jamison, LCSW

Behavioral Health Counselor

Lana Jamison, LCSW, is a licensed clinical social worker with over 20 years of experience. She completed her Bachelor of Science in Human Development & Family Studies at University of Missouri-Columbia in 1996, and received a Master’s Social Work from University of Kansas in 1998.

Life can be challenging - we are pulled in many directions and often give so much of ourselves to other people and the activities in our lives. It can feel overwhelming trying to balance it all. Lana works with individuals who regularly share concerns of exhaustion, worry, anger, stress, communication difficulties, resentment, and sadness. Engaging in a solution-focused, strengths-based approach to therapy can help us uncover and utilize the abilities we have and are likely already using in many areas of our lives. Sometimes taking the opportunity to discuss our concerns, priorities, and goals is an important step in getting our lives back to a more comfortable, positive place.

Lana is excited to serve Raytown School District employees and their families. She shares, “I look forward to learning about you and working together to create a plan for the improvements you desire and deserve.”

Jared Smiddy, PT, MPT

Physical Therapist

Jared Smiddy is an Illinois native who moved to Lee's Summit from Fort Collins, Colorado in 2021.  Jared has extensive experience in outpatient orthopedics and is certified in dry needling.  He is excited and honored to be able to serve Raytown School District employees.  

He completed his undergraduate and graduate degrees at Maryville University of Saint Louis, where he also played basketball and met his amazing wife, Jen, who is also a physical therapist.  Together, they have been blessed with two children, Jacob and Juliette, along with a Goldendoodle named Charlie.  

In his free time, Jared enjoys sports, music, the great outdoors, watching or coaching his kids' sports, and making memories with his family.

Support Staff

Tara Gilbert, LPN

Health Center Manager, Licensed Practical Nurse

Tara Gilbert obtained her Licensed Practice Nursing certification from Hillyards Technical School.  Throughout her time as a nurse, she has worked as a worker's compensation manager, Occupational Health Manager and has been in many leadership roles.  Tara is currently obtaining her RN degree and is enrolled to start her Master's program in Population Health Management in the fall of 2024.

LaNeisha Douglas, MA

Medical Assistant

LaNeisha is a Medical Assistant at the Raytown Schools Quality Care Clinic. She completed her education at Heritage College where she earned a certification as a Medical Assistant and X-Ray Technician. Prior to joining Premise Health, she was as Medical Assistant at KC Pain Center. LaNeisha strives to provide excellent medical service to all of her patients. She states, “I am devoted to giving excellent care to patients every day.”
